Mesoblast shares (ASX:MSB) have had quite the volatile few sessions, completely shifting gears to lead the ASX 200 today, having been the index laggard on Friday. With the 200 day SMA also recaptured on the day, on volume of 3.82 million shares (1.9x the ADV), bulls have decisively reclaimed the A$2.40 level.

Today’s move came after the company announced the completion of patient treatment in its pivotal Phase 3 trial for chronic low back pain, a milestone that brings the company one step closer to a potential US$10 billion market opportunity.

The Mesoblast share price rounded out the session at A$2.45, up 10.86% on the day, as markets digested the operational de-risking implicit in the trial update. The completion of treatment in all 350 enrolled patients removes a key execution risk from the bull case, and potentially clears a path to potential regulatory approval and commercialization.

What Was In The Mesoblast Report?

The company announced that it has completed patient treatment in MSB-DR004, its pivotal randomised controlled Phase 3 trial of rexlemestrocel-L for chronic low back pain associated with degenerative disc disease. A total of 350 patients were randomised and treated with either a single intradiscal injection of rexlemestrocel-L or a sham injection, up from an original target of 300 patients. Mesoblast attributed the increase to strong demand from trial investigators to enroll their patients in the program, a signal that markets interpreted as clinical confidence in the therapy’s mechanism and safety profile.

The trial’s primary endpoint aims to confirm the durable pain reduction at 12 months from a single injection observed in the earlier MSB-DR003 trial, which enrolled 404 patients and demonstrated long-term efficacy out to at least two years. Secondary endpoints include improvements in function, quality of life, and cessation of pain medication, including opioids. With 350 treated patients, Mesoblast stated that the trial is well-powered to show a greater treatment benefit compared with controls, addressing one of the key statistical concerns that have historically dogged the program.

Regulatory Pathway and Market Opportunity

Rexlemestrocel-L holds Regenerative Medicine Advanced Therapy designation from the U.S. Food and Drug Administration for treatment of chronic low back pain due to degenerative disc disease. The RMAT designation, granted in February 2023, provides eligibility for priority review once a Biologics License Application is filed, as well as access to rolling review and intensive FDA guidance throughout the development process. This regulatory fast-track is particularly significant given that chronic low back pain caused by inflammation and degenerative disc disease affects more than seven million people in the United States alone.

Mesoblast has consistently highlighted the commercial scale of the opportunity, estimating potential peak annual revenue exceeding US$10 billion even with only single-digit market penetration. The indication accounts for roughly 50% of prescription opioid use in the U.S., positioning an effective, durable, opioid-sparing therapy as highly attractive to regulators, payers, and potential commercial partners.

The company has guided that top-line results are expected in mid-calendar-year 2027, after the last treated patient completes 12 months of follow-up, with a Biologics License Application submission targeted for the third quarter of 2027 and potential approval around the second quarter of 2028 if results are positive.

Path To Monetisation

With commercial launch only feasible in 2028 at the earliest, Mesoblast faces a multi-year funding gap before potential revenue generation.

The company has historically relied on a combination of equity raises, partnerships, and debt facilities to finance its operations, and markets widely expect additional capital-raising activities or strategic partnerships ahead of a potential BLA submission. This financing overhang remains a persistent concern for holders, particularly given the company’s track record of delays and regulatory setbacks across its broader pipeline.

Despite these challenges, the CLBP program has emerged as Mesoblast’s core value driver, with management repeatedly emphasising the alignment with FDA on pivotal design, the strength of the earlier efficacy signal, and the scale of the unmet medical need.

The Setup

Mesoblast shares moved within 2 cents of the TTM break even line following today’s move, although perhaps more crucially, broke decisively above the 200 day SMA at A$2.31. There has been plenty of friction between the A$2.40-$2.50 price zone, and a move above in the coming days could well set bulls in motion. Looking to the analyst community, and the average price target of A$3.93 suggest as a substantial potential upside from here, yet the path is unlikely to be straight forward, and heavily leveraged to the outcome of MSB-DR004.

Bulls point to the massive addressable market, the opioid-sparing public health narrative, and the privileged regulatory status as reasons to remain engaged, while bears highlight the long wait for data, the risk of nuanced or marginal efficacy signals, and the likelihood of dilutive financing events before commercialization.

The index lagging performance on Friday came against the backdrop of a report that State Street now holds 5.1% of Mesoblast, raising fears of share lending and potential short selling. Today’s move on a trial update is a clear data point that has overshadowed those potential fears on the day. The bounce from index laggard to leader in back to back sessions underscores the level of volatility in the stock, so careful risk management is vital for anyone considering, or already holding MSB.

Bull Case:

  • 350-patient trial well-powered to detect treatment effect if present
  • RMAT designation provides priority review and rolling BLA submission pathway
  • US$10 billion peak revenue potential in massive opioid-linked chronic pain market
  • Completion of treatment removes key execution risk and anchors mid-2027 data timeline
  • Strong investigator demand signals clinical confidence in mechanism and safety profile

Bear Case:

  • Mid-2027 data readout leaves over one year of binary clinical risk
  • Chronic pain is subjective and placebo-sensitive, raising trial failure probability
  • Multi-year funding gap likely requires dilutive equity raises or partnerships
  • Prior MSB-DR003 trial failed to secure approval despite positive signals
  • Nuanced efficacy or safety findings could complicate regulatory pathway even if positive
