4DMedical shares (ASX:4DX) head into June of the back of a dramatic 18.86% rally in the final session of May, as markets digested a landmark three-year commercial agreement with Arizona-based SimonMed Imaging that opens the door to large-scale US outpatient deployment of its CT:VQ lung imaging technology.

The 4DMedical share price finished the month down approximately 1%, a stark contrast to the 1,000% gain recorded over the trailing twelve months. The late-month surge came after 4DMedical announced that SimonMed Imaging, one of the largest physician-owned outpatient radiology networks in the United States, had committed to rolling out CT:VQ and Lung Density Analysis (LDAf) across more than 170 imaging centres spanning ten states.

The agreement marks 4DMedical’s formal entry into the US outpatient market, a significant strategic shift that complements its existing footprint in academic hospitals and major health systems. Under the commercial terms, CT:VQ will be deployed immediately with revenue generation from day one, rather than through extended pilot programmes that have characterised some earlier hospital partnerships.

CT:VQ is a functional lung imaging technology that transforms routine chest CT scans into ventilation-perfusion studies without requiring contrast agents or nuclear medicine infrastructure. The technology received FDA 510(k) clearance in late 2025 as the first and only CT-based VQ imaging platform, positioning 4DMedical as a first-mover in an emerging category of respiratory diagnostics.

The SimonMed partnership is structured to generate real-world utilisation and reimbursement data, a critical factor for broader payer adoption across the fragmented US healthcare system. High-volume outpatient networks like SimonMed represent a materially different value proposition compared to single academic centres, with the potential to drive scan volumes at scale if clinical workflows and reimbursement pathways prove robust.

Operational Momentum

4DMedical’s commercial momentum has been building across multiple fronts. In January 2026, the company raised approximately A$150 million from institutional investors to accelerate CT:VQ commercialisation, strengthening its balance sheet ahead of the US rollout. The capital injection followed a series of reference-site wins, including an expanded commercial agreement with University of Chicago Medicine that included prepaid scan packages.

Regulatory clearances have also expanded the addressable market. In March 2026, CT:VQ received CE Mark approval, enabling commercial deployment across the European Union and adding a second major geographic pillar to the growth story. Australian partnerships, including multi-year contracts with Spectrum Medical Imaging for national lung cancer screening programmes, continue to provide a steady base of recurring revenue.

Despite the positive newsflow, valuation remains a contentious topic. With the 4DX share price up tenfold over twelve months and market cap at A$2.36 billion, 4DMedical is trading on expectations of rapid adoption across respiratory imaging applications including COPD, interstitial lung disease, long-COVID assessment, and lung cancer screening. Current revenue, however, remains modest relative to the market cap, leaving execution risk firmly in focus.

What Could June Hold For 4DX?

4DMedical’s end-of-May performance highlights the tension between transformational commercial progress and stretched valuation that defines the current investment debate. The A$4 level looms as a near term level of resistance, and how the share price behaves at this level could well set the tone for early June. A firm break and hold on strong volume, and bulls could be away. A breakdown to support levels could well see more range bound trading ahead.

The SimonMed deal represents a genuine inflection point in US market access, but whether it translates into the revenue ramp required to justify a ten-bagger valuation will depend on flawless execution in the quarters ahead. Markets are clearly willing to give management the benefit of the doubt for now, but the bar for delivery is once again higher.

Bull Case:

  • SimonMed deal validates scalability across 170-plus high-volume US outpatient centres
  • First-mover advantage with only FDA-cleared CT-based VQ technology strengthens competitive moat
  • A$150 million capital raise provides multi-year runway to fund commercialisation and evidence generation
  • Global regulatory clearances (FDA, CE Mark) unlock US and EU markets simultaneously
  • Long-term respiratory imaging tailwinds remain under-penetrated across COPD, IPF, and screening applications

Bear Case:

  • Valuation stretched after 1,000% twelve-month rally; high expectations leave little room for error
  • Revenue disclosure remains limited; per-site economics and utilisation rates unproven at scale
  • Execution risk elevated across multiple geographies, customer types, and reimbursement pathways simultaneously
  • Dilution from A$150 million raise pressures per-share metrics if revenue growth disappoints
  • Technical overhang from ASX 200 inclusion and profit-taking may amplify downside volatilit
